Eligibility

Make the most of Lennox’s available benefits options and cover the people during enrollment.

Employee Eligibility

Lennox benefits are available to those who are:
  • Salaried: Scheduled to work at least 20 hours per week
  • Hourly: Scheduled to work at least 30 hours per week

Dependent Eligibility & Verification

Depending on your eligibility and enrollment status, the following family members may also be covered.

If adding a new dependent in Lennox health coverage, you MUST upload dependent verification documents (marriage certificate, birth certificate, adoption papers) in BenefitSource within 60 days of enrollment.

  • A new dependent is someone who enrolls under your plan for the first time.
  • Verification for coverage is required only once, and you do not have to send documentation every year.

Call (800) 284-4549 to check the status as dependent coverage will not be effective until it’s approved.

Does the Working Spouse Surcharge Apply to You?

Regardless of your benefits status, all Lennox employees MUST answer these questions each year in BenefitSource.

If these questions are skipped, you will automatically receive the surcharge.

Certify Working Spouse Status

A $100 monthly Working Spouse surcharge will apply if you enroll a working spouse who is offered health coverage through their employer.

If this does not apply, you MUST confirm your current status by answering “No” on the Working Spouse Status question in BenefitSource during Open Enrollment.

In order to avoid this surcharge, it is necessary to answer this question regardless of whether you are enrolled in the medical plan.

If this question is skipped, you will automatically receive the surcharge.

NOTE: If your spouse’s status changes, call (800) 284-4549 and submit a Working Spouse Affidavit.*
