If you have been diagnosed with one of the conditions above, you may qualify for FREE condition-specific medications through Quantum Health’s $0 Copay Prescription Program. This benefit provides full cost coverage for certain approved medications (no copay required), but you MUST complete 100% of the required care path activities each year to remain eligible.
